Centennial is a Residential Treatment Home, serving two adults who have a mental illness and are ready and appropriate for living in an integrated community setting with the right supports and treatment services. The home, located in beautiful east Portland, is conveniently located near amenities such as stores, restaurants and entertainment. But residents are able to enjoy a quite atmosphere as it apart of a charming neighborhood that is nicely tucked into an old forested area.

Prior to admission, potential residents are screened for appropriateness by ColumbiaCare/Centennial staff, Multnomah County and the State of Oregon Addiction and Mental Health Division. All parties must agree that the proposed resident is likely to feel safe and hopeful about the placement, fits in with the other resident(s) of the home, is likely to benefit from the therapeutic environment and service offered within the home.

Centennial has an awake staff that work 24 hours a day, 7 days a week with supervision provided by a Qualified Mental Health Professional.  For more information about the Centennial please contact the Facility Administrator.
