HOSPITAL ASSESMENT & DIVERSION
ColumbiaCare understands that diverting individuals from unnecessary hospitalization can be extremely clinically beneficial for a person experiencing a mental health crisis, and provides significant cost savings to the service system. Our ICM staff are available on-call to respond when a Multnomah County individual is experiencing a psychiatric crisis and has been taken to a hospital. ColumbiaCare staff provide assistance upon request by performing a mental health assessment and review to determine what the clinical and safety needs of the member are; and whether or not they can be safely treated in a less intensive and medically necessary level of care as an alternative to inpatient hospitalization.
In coordination with Multnomah County and hospital staff, ColumbiaCare may be able to help the member immediately access community-based care such as a crisis resolution and/or respite center, or a temporary adult foster care environment until stabilized and able to return to their own home.
