SWING LANE
Swing Lane (est. 2018) is a 7-unit, Permanent Supportive Housing Program providing affordable housing to Jackson Care Connect Members living with a mental health disability. The goal of this program is to support individuals in order to be successful in their housing. This program is conveniently located near public transportation, and just minutes from a variety of community resources.
Swing Lane offers 5-units (one unit is completely ADA accessible) of Intensive Supportive Housing with the goal of helping individuals maintain an affordable independent living situation in the open housing market. Awake staff are onsite 8am-midnight, with on-call coverage all other hours, and the Supportive Housing Supervisor/QMHP is on-site 40 hours a week. Flexible, wraparound services such as case management, skills training, supported employment and other outpatient services are tailored to the individual’s needs, and are available in the community. Clients are supported by on-site programming, which includes daily contact and support from staff.
Swing Lane also offers two 3-bedroom units (which can be shared as appropriate) offering more Transitional Supportive Housing where services are provided to prepare individuals for more permanent, independent living. Room and board services are provided 7 days a week, which includes the following:
In addition to providing mental health treatment and habilitative care, ColumbiaCare acts as a mental health-friendly landlord. Our Property Management division carries out all property management functions. All apartments qualify for HUD funding. Tenants are responsible for their rent (or a negotiated portion thereof) and all utilities. All tenants are screened for clinical appropriateness both by ColumbiaCare and Jackson Care Connect.

- Meal preparation (3 meals a day)
- Cleaning Services
- Medication Support (an attempt to give or encourage the individual to take their medication)
- Medically Necessary Services
- Skills Training
- 24/7 On-Call Staff for Emergencies
