ASTER PLACE
Aster Place (east. 2014) is a 2-unit, non-licensed, Supportive Housing program located in Tigard, Oregon. ColumbiaCare has contracted with Washington County to provide exclusive use of in-home, transitional services at Aster Place for their clients who have a mental health concern, who need temporary or transitional housing and support services, but who are not in need of hospital or Crisis Resolution Center level of care. Tenants are identified and referred through Washington County, and admission to Aster Place is a collaborative process between the two agencies.
ColumbiaCare provides move-in ready, individual bedrooms with use of the common areas of the home. Unless otherwise arranged, tenants provide their own food, clothing, transportation, personal items, and medication. ColumbiaCare staff provide 4 periodic health and safety visits per 24 hour period that, whenever possible, coincide with medication schedules. ColumbiaCare provides a layer of safety and supervision--through documented visits, as well as on-call availability. All concerns, issues, and/or emergencies are reported to Washington County as soon as reasonably possible.
